Iowa requires every physical therapist to hold an active state license before practicing in the state. The Iowa Board of Physical and Occupational Therapy — within the Iowa Department of Inspections, Appeals, and Licensing (DIAL) — administers all PT licensure under Iowa Code Chapter 148A and 481 IAC Chapters 800 through 803. All applications are submitted online through the Iowa DIAL Licensure Portal.
Iowa’s application process is relatively streamlined compared to many states. There is no state jurisprudence examination, and the pathway from CAPTE-accredited program to NPTE to Iowa license is well-defined. The $120 application fee is combined with a separate $55 criminal background check fee totaling $175 for most applicants. Iowa joined the PT Compact, allowing eligible out-of-state PTs to obtain Iowa compact privileges and Iowa PTs to purchase compact privileges in other member states at a $60 state fee.
One renewal planning note worth flagging at the application stage: Iowa PT licenses expire on the 15th day of the licensee’s birth month every two years — not a fixed statewide date. New licensees should confirm their personal expiration date and begin tracking CE from day one of their license period.
Iowa and the PT Compact
Iowa is an active member of the Physical Therapy Licensure Compact. The compact allows eligible PTs from other member states to obtain an Iowa compact privilege without applying for a full Iowa license. Iowa PTs may also purchase compact privileges in other participating states. The state fee for an Iowa compact privilege is $60, collected by the Physical Therapy Compact Commission. This fee is waived for active duty military members and qualifying spouses under 481 IAC 800.3.
PTs practicing in Iowa under a compact privilege are subject to Iowa’s practice rules under 481 IAC Chapters 801 and 802. Compact privileges expire when the home-state license expires. Verify current compact member state participation and procedures through the PT Compact Commission, as membership can change.
Iowa PT License Requirements
Education
Under 481 IAC 800.5, applicants must have graduated from a physical therapy program accredited by a national accreditation agency approved by the Board — in practice, a CAPTE-accredited program. The DIAL application page specifies that degrees granted on or before January 31, 2004 must be equivalent to a bachelor’s degree; degrees granted on or after February 1, 2004 must be a master’s degree or higher (i.e., DPT or equivalent). Official copies of academic transcripts must be submitted directly from the school to the Board. No application will be reviewed until official transcripts have been received. An application remains active for two years from the date received; if all materials are not submitted within that period, a new application and fee are required.
Foreign-Educated Applicants
Foreign-educated applicants who do not hold a license in another US state or territory must submit an English translation and credentials equivalency evaluation through the Foreign Credentialing Commission on Physical Therapy (FCCPT) using the current FSBPT Coursework Tool. The professional curriculum must be equivalent to CAPTE standards. If English was not the language of instruction and textbooks, applicants must also achieve a TOEFL iBT total score of at least 89 with accompanying minimum component scores (24 writing; 26 speaking; 21 reading; 18 listening). Foreign-educated applicants holding a license in another US state or territory may apply by endorsement.
NPTE
Under 481 IAC 800.4, passage of the National Physical Therapy Examination (or other Board-approved nationally recognized equivalent) is required. Iowa follows FSBPT policies regarding NPTE eligibility and attempt limits — the Board will not approve an applicant for testing once FSBPT confirms all lifetime opportunities have been exhausted.
Criminal Background Check
Under 481 IAC 800.2(4), all initial applicants must submit a completed fingerprint card and signed waiver form to facilitate a national criminal history background check through the Iowa Division of Criminal Investigation (DCI) and the FBI. The cost of the check is assessed to the applicant ($55). Criminal history is reviewed on a case-by-case basis.
Licensure by Endorsement
Physical therapists currently or previously licensed in another state may apply for Iowa licensure by endorsement under 481 IAC 800.2. Endorsement applicants must provide verification of their license from the most recently licensed jurisdiction, sent directly from that jurisdiction to the Iowa Board. Web-based verification may substitute if it includes the licensee’s name, initial licensure date, current status, and any disciplinary actions. Endorsement applicants who have been licensed in another state must also demonstrate one of: completion of 40 CE hours in the preceding two years; active practice of at least 2,080 hours in the preceding two years; full-time faculty service in an accredited PT program for at least one of the preceding two years; or passage of the NPTE within two years before application.
How to Apply for an Iowa PT License
- Submit application online. Complete the application through the Iowa DIAL Licensure Portal and pay the $120 application fee. All fees are nonrefundable.
- Submit official transcripts. Have your physical therapy program send official transcripts directly to the Iowa Board. No application will be reviewed until transcripts are received.
- Complete the fingerprint background check. Submit the required fingerprint card and signed waiver form. The $55 fee is assessed to the applicant. Contact the Board for current fingerprinting instructions.
- Arrange NPTE score transfer. Register for the NPTE through FSBPT and have scores reported directly to the Iowa Board.
- For endorsement applicants: submit license verification. Request verification of your current or most recent state license, sent directly from that state’s board to Iowa — or use web-based verification if it contains the required elements.

- No jurisprudence exam required. Iowa does not require a state jurisprudence examination at initial licensure or at renewal — a notable simplification compared to neighboring states.
- Birth-month expiration means your renewal date is personal to you. Unlike states with a fixed December 31 or March 1 deadline, Iowa’s renewal date is tied to your individual birth month. Mark your calendar — the Board’s failure to send a notice does not relieve you of the responsibility to renew on time.
- Endorsement applicants: CE or practice verification required. Iowa requires evidence of recent PT activity from endorsement applicants — either 40 CE hours, 2,080 hours of practice, or equivalent activity in the preceding two years. Have these records organized before applying.
