West Virginia licenses occupational therapists through the Board of Occupational Therapy (WVBOT), with applications and renewals both handled through Certemy, the Board’s online licensing platform. West Virginia is an active OT Compact member state — it has completed the technical integration required to actually issue compact privileges, rather than having only enacted compact legislation.
See the summary table above for the current fee and pathway details, and the section below for what makes West Virginia’s process distinct from other states.
What Makes West Virginia Different
- West Virginia is already issuing OT Compact privileges — not just enacted. Many states in this database have passed compact legislation but are still working toward operational status. West Virginia has completed that integration, meaning compact privileges from West Virginia are functional today, not pending a future milestone.
- The fingerprint-based background check applies to compact eligibility, not standard licensure. If you’re applying for a straightforward West Virginia license without using the compact, you’re not going through FBI fingerprinting — that requirement is specific to purchasing an OT Compact privilege.
- OT and OTA licenses expire on different, fixed calendar dates. Rather than a birth-month system, every OT license in West Virginia expires December 31, and every OTA license expires June 30, of the applicable renewal year — the same dates for every licensee of that type, regardless of when they were originally licensed.
- The limited permit and the temporary license are two genuinely different things. A limited permit (90 days, non-renewable) is for new graduates awaiting NBCOT exam results. A temporary license is a separate mechanism for people already licensed and in good standing elsewhere under equivalent standards. Don’t assume these are the same pathway with different names.
Occupational Therapy Compact Status
West Virginia is a member of the Occupational Therapy Licensure Compact and — unlike a number of other member states — has completed the integration work needed to actually issue compact privileges. According to the Board’s own meeting minutes, West Virginia was one of the first six states to launch (alongside Minnesota, Ohio, Indiana, Virginia, and Maryland), and as of late April 2026 had already processed eleven incoming compact privileges to practice in West Virginia, with four West Virginia practitioners having completed the required FBI background check to become eligible for outbound privileges in other active states. Occupational therapists holding a compact privilege from another actively-issuing state can use it to practice in West Virginia today. In addition to the fees paid for a home-state license, purchasing a compact privilege requires an FBI criminal background check, a $75 fee assessed by the Occupational Therapy Compact Commission, and West Virginia’s own state privilege fee. Compact privileges are tied to your home-state license, do not renew automatically when that license renews, and must be separately re-purchased through the Compact each time.
Initial Licensure Requirements
WVBOT offers several distinct routes to a West Virginia OT license.
Initial Application (Examination)
For applicants who have completed an ACOTE-accredited occupational therapy program and passed the NBCOT exam. Requires a completed online application through Certemy, verification of your education and fieldwork, NBCOT results, a recent photo, and the applicable fee. Reporting your Social Security number is mandatory under state law.
Licensure Recognizing an Out-of-State License
West Virginia recognizes licenses held in other jurisdictions under both a general state reciprocity statute and provisions specific to occupational therapy, for applicants whose out-of-state license is in the same discipline and at an equivalent practice level.
Temporary License
A separate pathway for occupational therapists or occupational therapy assistants currently licensed and in good standing in another jurisdiction whose standards the Board considers equivalent to West Virginia’s own.
Limited Permit
Available to applicants who have completed their education and fieldwork requirements but are still awaiting NBCOT exam results. Valid for 90 days, non-renewable, and practice is limited to direct, close supervision by a currently licensed West Virginia occupational therapist. The permit expires immediately if the holder is notified of a failing exam score.
How to Apply for a West Virginia Occupational Therapist License
- Confirm which pathway applies to you (initial application, out-of-state recognition, temporary license, or limited permit) using the requirements above.
- Create or access your Certemy account and begin your application online.
- Arrange for your education and fieldwork verification, and your NBCOT results, to be submitted to the Board.
- Upload a recent photo as part of your application.
- Pay the applicable fee electronically through the State of West Virginia’s e-Gov payment processor.
- Once approved, your license number is assigned and a digital license card becomes available in your Certemy wallet; the Board also mails a physical Certificate of License and license cards.
Practical Notes
- Have your education, fieldwork, and NBCOT verifications sent directly from the issuing organization to the Board rather than routed through you — self-submitted documents are a common source of delayed applications.
- If applying for a limited permit, keep the 90-day, non-renewable clock in mind — there’s no second chance if you don’t pass the exam or complete your full application within that window.
- If you’re relocating and considering West Virginia’s temporary license pathway rather than standard out-of-state recognition, confirm with the Board which specific route applies to your situation — the two mechanisms have different underlying requirements even though both involve an existing out-of-state license.
- If you’re planning to use OT Compact privileges, budget for the FBI background check as a separate step from your West Virginia license application — it isn’t part of standard single-state licensure.
