Oklahoma requires every physical therapist to hold an active state license before practicing within the state. Licensure is administered by the Oklahoma State Board of Medical Licensure and Supervision through its Physical Therapy Advisory Committee, under the Oklahoma Physical Therapy Practice Act (59 O.S. §§887.1–887.18) and OAC Title 435, Chapter 20. Oklahoma’s application process includes the NPTE along with an Oklahoma-specific jurisprudence examination covering the state’s PT Practice Act and Board rules. Oklahoma joined the PT Compact on January 2, 2020, and currently active Oklahoma licensees may purchase compact privileges in other participating member states. Out-of-state PTs with compact-eligible licenses may practice in Oklahoma under compact authority, with the jurisprudence requirement applicable to compact privilege applicants as well.
Oklahoma and the PT Compact
Oklahoma became an active PT Compact member state on January 2, 2020. The compact allows licensed PTs from other member states to obtain Oklahoma practice privileges without applying for a full Oklahoma license, and Oklahoma PTs may purchase compact privileges in other participating states.
To be eligible for compact privileges, Oklahoma PTs must hold an active, unencumbered Oklahoma license with no disciplinary actions in the past two years. Applicants for compact practice privileges in Oklahoma must also complete the jurisprudence examination under OAC 435:20-3-6 and pay applicable compact fees. Oklahoma PT License Requirements
Education
Applicants must have graduated from a school of physical therapy approved by a national accrediting body recognized by the Board — in practice, this means a CAPTE-accredited physical therapy program. The examination application must document that the applicant has completed the required professional PT education.
NPTE
Successful passage of the National Physical Therapy Examination administered through FSBPT is required for licensure by examination. For endorsement applicants, NPTE scores from a nationally accepted standardized examination must be submitted through the Interstate Reporting Service or another recognized reporting service. All endorsement applicants must meet Oklahoma’s passing score standard — if the prior examination score does not meet Oklahoma’s requirement, the applicant must retake the examination.
Oklahoma Jurisprudence Examination
Oklahoma requires applicants to complete a jurisprudence examination as part of the licensure process under OAC 435:20-3-6. The examination is prepared by the Board and focuses on the Oklahoma Physical Therapy Practice Act and related Oklahoma Administrative Code. Criminal Background Check
All applicants for Oklahoma PT licensure are required to complete a biometric criminal background check. After the initial application is received, the Board emails applicants a Service Code to complete the background check process. Criminal history disclosure is also required in the application itself. Lawful Presence Documentation
All applicants for licensure by endorsement or examination — and reinstatement applicants — must submit an Evidence of Status Form verifying citizenship or qualified alien status. Licensure by Endorsement
Physical therapists licensed in another US state, the District of Columbia, or Puerto Rico may apply for Oklahoma licensure by endorsement under OAC 435:20-3-4, provided the written examination and grade standard on which the prior license is based are acceptable to the Board. If an endorsement applicant has not been employed as a physical therapist during the year prior to application, the Board may require a personal interview with a Board or Committee member. Applicants who are not eligible for endorsement may sit for licensure by examination upon payment of applicable fees.
Temporary Permit
Under 59 O.S. §887.10, the Board may issue a temporary permit to practice physical therapy for a period not to exceed one year to an applicant who meets the requirements to take the examination. The temporary permit expires when the applicant receives notice of examination results — passing or failing. The permit fee is $25.
Foreign-Educated Applicants
Foreign-educated physical therapists face substantial additional requirements under OAC 435:20-3-1.1, including a credentials equivalency evaluation by a Board-approved professional education credentials service, and verification of having completed an 800-hour (at least 120 days) interim supervised clinical practice period under continuous and immediate supervision of an Oklahoma-licensed PT. The supervised practice must be completed in Oklahoma at a facility serving as a clinical education site. How to Apply for an Oklahoma PT License
- Apply online through the Oklahoma Medical Board portal. Submit your application at medicalboard.ok.gov and pay the $100 application processing fee. All fees are non-refundable.
- Complete the biometric background check. After your application is received, the Board will email you a Service Code to complete the required background check. Do not delay — follow the instructions in that email promptly.
- Complete the jurisprudence examination. Access study materials and examination information at okmedicalboard.org/resources. Confirm current requirements with the Board.
- Arrange NPTE score transfer. Have FSBPT submit your NPTE scores to the Oklahoma Board through the Interstate Reporting Service or FSBPT Score Transfer Service.
- Submit all required documentation. Include educational transcripts, prior jurisdiction verifications (for endorsement applicants), and any other materials required by the Board. Confirm document submission requirements with the Board at (405) 962-1400.
- Pay the license fee. Upon application approval, pay the $50 license fee. Payment methods accepted are credit card, debit card, and EFT from checking or savings account.

- Annual renewal begins immediately. Oklahoma PT licenses expire January 31 of each year, renewed during January. If your license is issued in, say, October, your first renewal comes just three months later. Budget the $90 renewal fee and plan CE accordingly from day one.
- Jurisprudence exam is part of the process. Study materials are available on the Board’s resources page. Confirm current format and timing requirements with the Board before starting your application — procedures can be updated.
- Endorsement applicants: check exam score requirements. Oklahoma applies its own passing score standard to endorsement exam scores. If your prior NPTE score does not meet Oklahoma’s requirement, you may need to retake the examination. Verify this with the Board before applying by endorsement.
- Foreign-educated applicants: the process is substantial. Oklahoma requires an 800-hour supervised clinical practice period that must be completed in Oklahoma. Plan for a timeline of six months or more before licensure is possible. Contact the Board early in the process.
