Wisconsin requires all practicing physical therapists to hold a current Physical Therapist License issued by the Physical Therapy Examining Board, which operates under the Wisconsin Department of Safety and Professional Services (DSPS). All applications are submitted through Wisconsin’s LicensE portal at license.wi.gov. Wisconsin offers several licensing pathways, including Licensure by Examination, Licensure by Endorsement, PT Compact Privilege, Temporary License, and Locum Tenens License.

Wisconsin has two notable examination requirements beyond the NPTE. First, the NPTE registration must be filed with FSBPT at the same time the Wisconsin application is filed — not before or after, but simultaneously. Second, all applicants must pass the Wisconsin Statutes and Rules Exam, an online open-book examination on Wisconsin PT laws and administrative code. This exam is only available after DSPS has received and processed the application — applicants should not expect to take it immediately after applying.

Wisconsin also requires fingerprints and a recent photograph for all applicants under Wis. Stat. §440.03(13)(c), making Wisconsin’s background check requirements more extensive than states that only require disclosure questions. The application fee is collected through the LicensE portal — verify the current amount at license.wi.gov before applying, as Wisconsin periodically adjusts fees.

What Makes Wisconsin Different

Wisconsin stands out for its simultaneous NPTE and state application filing requirement. Unlike most states where these are sequential, Wisconsin requires the NPTE registration to be filed with FSBPT at the same time the Wisconsin application is submitted to DSPS. New graduates should coordinate both filings carefully to avoid delays.

Wisconsin also has an oral examination provision. The Board may require an oral examination in circumstances authorized by Wis. Admin. Code PT 2. Applicants with any of the following backgrounds should be aware this provision exists and may apply: prior conviction of a substantially related crime, adverse action by another licensing authority, extended period of inactivity, chemical substance use affecting practice, or graduation from a non-Board-approved school. If the Board determines an oral exam is required, the applicant will be advised and an additional fee applies. Not all applicants with these backgrounds will be required to take an oral exam — the Board reviews each case individually.

Additionally, Wisconsin offers a fee reduction program for low-income applicants under Wis. Stat. §440.03(11m). Eligible applicants may pay 10% of the initial credential fee rather than the full amount, subject to verification with the Wisconsin Department of Revenue. Form 3217 must accompany the application to claim this reduction.

PT Compact and Wisconsin

Wisconsin is a PT Compact member state under Wis. Stat. §448.985. Wisconsin’s Compact membership enables eligible out-of-state PTs to practice in Wisconsin through a compact privilege without obtaining a full Wisconsin license.

If Wisconsin is your home state:

Wisconsin-licensed PTs in good standing may purchase Compact Privileges to practice in other member states through the PT Compact Commission at ptcompact.org. Your Wisconsin license serves as your home state license for Compact purposes.

If you are licensed elsewhere and want to practice in Wisconsin:

You may apply for a Wisconsin Compact Privilege rather than a full Wisconsin license, provided your home state is a Compact member and your license is in good standing. Wisconsin charges an additional $56 state compact privilege fee beyond the PT Compact Commission’s fee. Wisconsin also requires passing the Wisconsin Statutes and Rules Exam before a compact privilege is issued — this is Wisconsin’s jurisprudence requirement for compact applicants. Apply through the PT Compact Commission at ptcompact.org.

For travel PTs:

Wisconsin’s PT Compact membership streamlines access for travel PTs. The $56 Wisconsin state fee and Wisconsin Statutes and Rules Exam requirement are important planning items for travel PTs pursuing Wisconsin compact privileges. Verify current member state status and Compact eligibility requirements at ptcompact.org/ptc-states.

Wisconsin PT Licensing Requirements

Wisconsin Statutes and Rules Exam

All applicants — examination, endorsement, and compact privilege — must pass the Wisconsin Statutes and Rules Exam. This is an online, open-book examination covering Wisconsin statutes and administrative code governing physical therapy practice. A passing score of 85% is required.

Timing differs by pathway. For compact privilege applicants, exam instructions are automatically emailed within 30 minutes of successful application submittal through LicensE. For examination and endorsement applicants, the exam is available after DSPS has received and processed the application — applicants receive exam access information from DSPS after their application is processed. For all pathways, official exam scores must be received and processed by DSPS and are not automatically updated upon exam completion. Applicants who fail may retake the exam upon payment of a reexamination fee.

NPTE — Simultaneous Filing Requirement

Examination candidates must pass the National Physical Therapy Examination (NPTE) administered by FSBPT. Wisconsin’s unique requirement is that NPTE registration must be filed with FSBPT at the same time the Wisconsin application is filed with DSPS. The NPTE registration form and fee go directly to FSBPT at fsbpt.org. NPTE scores are sent directly from FSBPT to DSPS — applicants are responsible for ensuring this designation is made.

Fingerprints and Photograph

All applicants must submit fingerprints and a recent photograph (head and shoulders only) via Form 2687, per Wis. Stat. §440.03(13)(c). The fingerprint authorization triggers an FBI criminal history background check. Applicants receive digital fingerprint instructions from DSPS after the Form 2687 is submitted. Fingerprint results may be used solely for licensing purposes and may not be disseminated outside the receiving department.

Education and Transcripts

Applicants must have graduated from a CAPTE-accredited physical therapy program approved by the Board. Proof of graduation is submitted via the Certificate of Professional Education (Form 3196), which must be sent directly from the institution to DSPS — the Board will reject forms received from the applicant. The Board also cannot accept anticipated graduation dates; the form must not be completed until the degree has been awarded.

Lawful Presence

Wisconsin does not require U.S. citizenship for PT licensure. However, DSPS tracks legal status for qualified aliens and nonimmigrant licensees lawfully present in the United States. If your legal status as a qualified alien or nonimmigrant has changed since your credential was issued or since your last renewal, contact DSPS at (608) 266-2112 or dsps@wisconsin.gov. A Social Security number is required to apply for a DSPS credential.

Name Consistency

If the name on any credential differs from the name on the application — for example, due to marriage or legal name change — a certified copy of the marriage certificate, divorce decree, or court order must be submitted with the application.

Temporary License

A Temporary License ($10) is available only for new graduates who are waiting to sit for the NPTE for the first time. The temporary license requires: a completed PT application, Certificate of Professional Education (Form 3196), passing of the Wisconsin Statutes and Rules Exam, and completed NPTE registration. Temporary license holders may practice only under the direct supervision of a Wisconsin-licensed PT.

Endorsement (Out-of-State Licensed PTs)

Out-of-state licensed PTs apply through the Endorsement pathway using an NPTE score transfer. Scores must be requested and forwarded to DSPS through FSBPT. Verification of licensure from every state in which the applicant has ever held a PT license must be sent directly from each state board to DSPS — verifications sent by the applicant are rejected. All other application requirements apply, including the Wisconsin Statutes and Rules Exam and fingerprinting.

Locum Tenens License

Wisconsin offers a Locum Tenens License for health care providers credentialed in good standing in another state who need a temporary credential while a permanent Wisconsin license application is pending. See the DSPS website for current locum tenens eligibility requirements.

Foreign-Trained Applicants

Foreign-trained PT candidates must obtain educational equivalency verification from a Board-approved evaluation service. Board-approved services include International Consultants of Delaware (ICD), Foreign Credentialing Commission on Physical Therapy (FCCPT), and International Education Research Foundation (IERF). All foreign-trained candidates must also pass the TOEFL examination — scores sent directly from ETS to DSPS. Foreign-trained candidates applying for late renewal (5+ years lapsed) are exempt from the education evaluation requirement.

  4. Request that your institution complete and send the Certificate of Professional Education (Form 3196) directly to DSPS — not through you

Practical Notes:
  • File NPTE registration with FSBPT and the Wisconsin application simultaneously — Wisconsin requires both at the same time, which is different from most states where these are sequential.
  • The Wisconsin Statutes and Rules Exam is only available after DSPS processes your application — you cannot take it before applying. Factor this into your timeline.
  • If your application has no activity for one year, it may be abandoned without notice. Keep your application moving forward.
  • Verify the current initial application fee at license.wi.gov before applying — Wisconsin periodically adjusts fees. The August 2025 DSPS fee schedule confirmed the renewal fee but initial application fees are collected at portal submission.
  • Low-income applicants may be eligible for a 90% fee reduction under Wis. Stat. §440.03(11m) — submit Form 3217 with your application.
